Nonoperative, Dynamic Treatment of Acute Achilles Tendon Rupture: Influence of Early Weightbearing on Biomechanical Properties of the Plantar Flexor Muscle–Tendon Complexd A Blinded, Randomized, Controlled Trial

J Foot Ankle Surg. 2015 Mar-Apr;54(2):220-6.

60 patients diagnosed with acute Achilles tendon rupture and conservatively treated through early mobilization and were randomized to undergo either early weightbearing or non-weightbearing protocols. The purpose of this study was to evaluate the impact of a weightbearing protocol on the recovery of ankle biomechanics when compared to a non-weightbearing protocol. Passive tension in the plantar flexor muscle-tendon complex, peak passive torque, energy stored, and strength were measured at follow-up visits at 6 and 12 months, and compared through Limb Symmetry Index (LSI). Stiffness during dorsiflexion was also evaluated and compared. The only significant difference noted at 12 month follow-up was lower stiffness at terminal dorsiflexion in the early weightbearing group versus the non-weightbearing group. All other outcome measures did not significant differ between groups at 12 months.
