A prospective randomized controlled study of hamstring and bone-free quadriceps tendons autografts in arthroscopic ACL reconstruction.

40 patients undergoing arthroscopic anterior cruciate ligament reconstruction (ALCR) were randomized to receive either a hamstrung tendon autograft (HT; n=20) or a bone-free quadriceps tendon autograft (QT; n=20). Outcomes of interest included thigh circumference, isokinetic quadriceps and hamstring peach torque, Lachman test, anterior drawer test, Lysholm knee score, International Knee Documentation Committee (IKDC), hamstring to quadriceps ratio (H/Q), and the incidence of adverse events. Extensor and flexor peak torque were significantly lower in the HT group after 2 years compared to preoperative values. The QT group also had a greater H/Q ratio after 2 years. IKDC, KOOS, and Lysholm scores were similar between the two groups post-operatively, but both groups showed signficant improvements from baseline to 2 years. The use of QT autografts may be an effective option for ACLR due to similar clinical outcomes as HT autografts after 2 years.
