Weight Loss in Patients Waiting for Total Hip Arthroplasty: Fiber-Enriched High Carbohydrate Diet Improves Hip Function and Decreases Pain before Surgery.
OrthoEvidence Journal (OE Journal) - ACE Report
OE Journal. 2021;9(20):5 J Clin Med. 2021 Sep 17;10(18): 4203.What this means for my practice?
The results of this study suggest that a 3 month fiber-enriched high carbohydrate diet can significantly reduce body weight and BMI whilst improving clinical outcomes scores prior to total hip arthroplasty surgery in obese hip osteoarthritis patients. This study was limited by selection bias: patients in the diet group had significantly higher HOOS scores compared to the control group. Moreover, adherence to the diet was not measured, and the small sample size may have prevented the detection of significant differences in outcomes. Future studies are required to further investigate the utility of fiber-enriched high carbohydrate dieting prior to total hip arthroplasty.
Resumen del estudio
Sixty-one obese patients with hip osteoarthritis were randomized to receive 3 months of a fiber-enriched high carbohydrate diet (n=36) or control (free diet; n=25) for the reduction of body weight, and improvement of pain and hip function prior to total hip arthroplasty. Outcomes of interest included weight, body mass index (BMI), cholesterol, Oxford Hip Scores (OHS), Hip Disabilities and Osteoarthritis Outcomes Score (HOOS), and the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC). No significant differences in weight, BMI, or OHS scores were observed between the diet and control groups. However, cholesterol levels and WOMAC scores were significantly in favour of the diet group at follow up. HOOS scores at both baseline and follow up were significantly different between the two groups.
