No need for hip precautions after total hip arthroplasty with posterior approach: A systematic review and meta-analysis.

Three randomized controlled trials including 1,215 patients undergoing total hip arthroplasty (THA) via the posterior approach were included in this systematic review and meta-analysis comparing no hip precautions (NHP) versus standard hip precautions (HP). Pooled outcomes of interest included hip dislocation rates, Hip disability and Osteoarthritis Outcome Score, joint replacement (HOOS JR) scores over 6 weeks, and time to discontinuation of mobility aids. The meta-analysis demonstrated no statistically significant difference in hip dislocation rates between the HP and NHP groups, while the NHP group showed significantly better functional outcomes with higher HOOS JR scores at 6 weeks, 3 months, and 12 months postoperatively and a faster discontinuation of mobility aids. These findings suggest that omitting postoperative hip precautions after THA via the posterior approach does not increase dislocation risk and may improve functional recovery and patient mobility.
