Study Summary

47 patients with lumbar disc herniation (LDH) were analyzed after randomization to treatment with a multifunctional traction device (MTD) combining traction and rotational manipulation or conventional lumbar traction. Both groups received six 20-minute treatment sessions over 2 weeks, while the MTD group additionally received alternating 15°–25° lumbar rotation during traction. The primary outcome of interest was disability measured using the Oswestry Disability Index (ODI). Secondary outcomes included the Japanese Orthopaedic Association (JOA) score, visual analogue scale (VAS) pain score, MRI-based Pfirrmann grading, disc protrusion severity, and adverse events. Outcomes were assessed immediately after treatment and at 3 and 6 months. Overall, the results of the study revealed greater improvements in ODI and longitudinal VAS outcomes with MTD, while JOA and MRI findings did not differ between groups. These findings suggested that adding rotational manipulation to lumbar traction may provide greater functional and pain-related benefits than conventional traction alone in patients with LDH.
