9 randomized controlled trials with 360 participants with chronic ankle instability (CAI) were included in this systematic review and meta-analysis in which patients either received hip strengthening or were part of the control group. The primary outcomes of interest were the Star excursion balance test (SEBT) for dynamic postural control and hip muscle strength. The secondary outcomes of interest were the functional outcomes with Cumberland ankle instability tool (CAIT) and Foot and ankle ability measure (FAAM)-Sports scores. Hip strengthening significantly improved SEBT posteromedial and posterolateral reach distances, hip abductor strength, and CAIT scores, but did not significantly improve anterior reach, hip extensor or external rotator strength, or FAAM-Sports scores. Overall, adding hip strengthening to ankle rehabilitation appeared beneficial for dynamic postural control, whereas isolated hip strengthening did not provide a clear additional benefit.
