Prospective randomized study on the effect of concurrent bone filling of tibial peg holes in cementless total knee arthroplasty.

66 (88 joints) patients undergoing cementless total knee arthroplasty (TKA) were randomized to receive either bone filling of tibial peg holes (48 joints) or no bone filling (40 joints). All randomized joints were included in the final analysis. The outcomes were plain radiographic findings (longitudinal trabecular thickening and reactive lines below the peg) and bone mineral density measured at five predefined tibial regions at 1, 3, 6, and 12 months. Overall, the bone filling group demonstrated significantly higher bone mineral density in key regions during the early postoperative phase and a significantly greater proportion of longitudinal trabecular thickening below the peg, suggesting improved initial fixation strength.
