Effects of dynamic neuromuscular stabilization training on the core muscle contractility and standing postural control in patients with chronic low back pain: a randomized controlled trial.

Sixty patients with chronic low back pain (CLBP) were randomized to receive dynamic neuromuscular stabilization (DNS) training (n=30) or conventional core exercises (n=30). The primary outcome of interest was change in core muscle contractility (percent change in transversus abdominis [TrA], diaphragm, multifidus). Secondary outcomes included standing postural control (center of pressure [COP] metrics under eyes-open and eyes-closed conditions) and clinical questionnaires (VAS, ODI, RDQ). Outcomes were assessed pre- and post-intervention across a 4-week program (3 sessions/week, 50 minutes/session). Overall, the results revealed greater increases in TrA and diaphragm contractility with DNS and superior standing postural control in the eyes-closed condition, while pain/disability improved over time in both groups without between-group differences. These findings suggest DNS offers added benefits for deep core activation and proprioceptive control beyond standard core exercise.
