Does Orthosis Improve Outcomes of Conservative Treatment in Trigger Fingers? A 3-Arm Prospective Randomized Controlled Trial.

108 patients representing 122 trigger fingers with Green grade 1–3 trigger finger were randomized to receive nighttime extension orthosis, cortisone injection, or combined orthosis plus cortisone injection. The primary outcome of interest was the incidence of triggering during 10 repeated grips at 6 weeks. Secondary outcomes included patient-reported symptom relief, Patient-Rated Wrist and Hand Evaluation (PRWHE) scores, need for additional treatment or surgical release, grip strength, range of motion (ROM), and complications. Outcomes were assessed at 6 weeks and at 3, 6, and 12 months, with an average follow-up of 29 months. Overall, the results of the study revealed no significant difference in triggering frequency between treatment groups at 6 weeks, and combined therapy did not significantly improve outcomes compared to injection alone. These findings suggest that orthosis and cortisone injection are similarly effective options and that combining treatments does not demonstrate an additional benefit.
