A Prospective, Randomized, Controlled Comparison of Adhesive Wound Closure Devices in an Orthopaedic Patient

Final skin closure with the Clozex device was significantly faster and resulted in favourable scarring compared to suturing, and the Clozex device may provide superior scar outcomes vs. the zipper at 3 months. Moreover, the device cost of Clozex was the lowest of the 3 interventions. This study was limited by the significant loss to follow-up (>50% attrition), the inclusion of mostly knee surgery patients, and longer average suture distance in the suture group.

Seventy-two patients undergoing orthopaedic surgery (32 completed follow-up and were included in the analysis) were randomized in this trial to receive either final wound closure using a suture (n=10), zipper (n=11), or Clozex surgical device closure (n=11). Outcomes of interest included skin closure time, surgeon satisfaction, scar evaluation on the Stony Brook Scar Evaluation Scale (SBSES) at 2 and 3 weeks post-surgery, patient satisfaction, and closure cost. Results revealed significantly faster closure and better SBSES scores in the Clozex group vs. suturing, as well as superior SBSES scores at 3 months vs. zipper closure. Zipper was significantly faster to apply than suture closure. No differences in patient satisfaction were observed between the two groups. The cost of closure was lowest in the Clozex group, followed by zipper closure and suturing.
