Comparison between Volar Locking Plate and Kirschner Wire Fixation for Unstable Distal Radius Fracture: A Meta-Analysis of Randomized Controlled Trials.
J Wrist Surg . 2023 Apr 14;13(5):469-480.Fourteen randomized controlled trials involving 1,450 patients with unstable distal radius fractures were included in this systematic review and meta-analysis comparing volar locking plate (VLP) fixation and Kirschner wire (K-wire) fixation. Pooled outcomes of interest included DASH scores, PRWE scores, grip strength, range of motion (ROM), radiographic parameters, complication rates, and reoperation rates. The analysis showed that VLP fixation was associated with improved DASH scores, grip strength, and ROM at early follow-up points but exhibited minimal differences at 12 months. Radiographically, VLP fixation resulted in better volar tilt but did not improve other parameters. Overall, VLP fixation offers early functional recovery advantages, but these benefits diminish over time, with both techniques being effective in long-term management.
