Randomized Controlled Study Comparing Hamstring Graft and Peroneus Longus Tendon Graft in Arthroscopic ACL Reconstruction.
J Orthop Case Rep . 2024 Nov;14(11):240-245.One hundred twenty patients with isolated ACL injuries were randomized to receive either a tripled or quadrupled PLT autograft (n=60) or a quadrupled HT autograft (n=60). The primary outcome of interest was functional improvement, measured using the International Knee Documentation Committee (IKDC) and Tegner–Lysholm scores. Secondary outcomes included graft diameter, graft harvesting time, donor-site morbidity, and time to return to sports. Outcomes were assessed preoperatively and at 3, 6, 12, and 24 months postoperatively. Overall, the results revealed that the PLT graft had a significantly larger mean diameter and required a shorter harvesting time compared to the HT graft. Patients in the PLT group returned to sports a mean of 36 days earlier than those in the HT group, reported lower donor-site morbidity, and demonstrated superior subjective functional scores at 24 months. These findings suggest that PLT grafts may provide superior outcomes in ACL reconstruction, though further research is needed.
