16 randomized controlled trials (RCTs) involving 3776 elderly patients with displaced femoral neck fractures were included in this systematic review and meta-analysis comparing cemented versus uncemented femoral stem fixation in patients undergoing hemiarthroplasty (HA) or total hip arthroplasty (THA). Pooled outcomes included overall mortality, periprosthetic fracture, dislocation, and wound infection. Cemented fixation significantly reduced overall mortality and periprosthetic fracture risk compared with uncemented fixation, with consistent findings across arthroplasty types. No significant differences were observed in dislocation or wound infection rates. These findings suggested that cemented femoral stems provided improved mechanical stability and were associated with fewer serious complications in elderly patients undergoing arthroplasty for displaced femoral neck fractures.
