Bone Marrow Aspirate Concentrate May Decrease Reoperation in Osteochondral Allograft Transplantation: A Prospective, Randomized, Double-Blind Investigation.

Thirty-six patients undergoing osteochondral allograft transplantation of the knee were randomized to receive bone marrow aspirate concentrate (BMAC) augmentation or a sham incision without BMAC. The primary outcome of interest was graft integration on computerized tomography (CT) imaging at 6 months. Secondary outcomes of interest included reoperation rates, cyst formation, and patient-reported outcomes such as the International Knee Documentation Committee (IKDC) score and Knee injury and Osteoarthritis Outcome Score - Joint Replacement (KOOS-JR) score. Outcomes were assessed at 6 months, 1 year, and 2 years postoperatively. Overall, the results of the study revealed that BMAC did not significantly improve graft osseous integration or patient-reported outcomes metrics (PROMs) but significantly reduced reoperation rates. These findings suggest that BMAC augmentation may improve graft durability despite similar imaging and functional outcomes.
