120 patients undergoing total knee arthroplasty for knee osteoarthritis were randomized to receive either barbed knotless sutures (n=60), continuous sutures (n=60), or interrupted sutures (n=60). Outcomes of interest included pain on a Visual Analog Scale (VAS), range of motion, Knee Society Score (KSS), Oxford Knee Score (OKS), continuous passive motion (CPM), maximum CPM angle, time to capsule closure, and the incidence of adverse events. There were no significant differences in pre- and postoperative ROM, KSS, OKS, and CPM outcomes in all three groups. A significant difference in VAS pain scores was noted between groups on postoperative days 3 and 10, and 12 weeks, particularly showing higher mean VAS scores in the continuous suture group compared to the barbed and interrupted suture group. There were no significant differences in the incidence of complications between all three groups.
