Aspirin versus low-molecular-weight heparin for thromboprophylaxis after orthopaedic surgery: a systematic review and meta-analysis.
Blood Coagul Fibrinolysis. 2024 Apr 18;35(4):187–195.Six randomized controlled trials including 23,327 patients who underwent orthopaedic surgery were included in this systematic review and meta-analysis comparing aspirin vs. low-molecular-weight heparin (LMWH) for thromboprophylaxis. Pooled outcomes of interest included venous thromboembolism (VTE), deep vein thrombosis (DVT), pulmonary embolism (PE), major bleeding, wound complications, wound infection, and death. LMWH was significantly more effective than aspirin in reducing VTE and DVT events. However, no significant differences were found between the two treatments regarding PE, bleeding, wound complications, infection, or mortality. These findings suggest LMWH is more effective for VTE prevention without compromising safety.
