Combined Arthroscopic Wafer Procedure and Triangular Fibrocartilage Complex Debridement versus Ulnar Shortening Diaphyseal Osteotomy in Management of Ulnar Impaction Syndrome: A Randomized Clinical Trial.

Forty-three patients with ulnar impaction syndrome and less than 4 mm positive ulnar variance were randomized to undergo either arthroscopic wafer procedure combined with TFCC debridement (n=21) or diaphyseal ulnar shortening osteotomy (n=22). The primary outcome was the Modified Mayo Wrist (MMW) score. Secondary outcomes included DASH score, operative time, complications, and patient satisfaction. Outcomes were assessed over a minimum of 12 months. Overall, the study found that the AWP group had significantly better MMW and DASH scores, shorter operative times, and fewer complications than the USO group. These findings suggest that AWP with TFCC debridement is a safer and more effective surgical option for managing UIS with minor ulnar positive variance.
