Are positional screws superior to lag screws in fixation of medial malleolar fractures? A randomized controlled trial.
Eur J Orthop Surg Traumatol. 2024 Dec;34(8):3897-3902.Sixty-eight patients with isolated closed medial malleolar fractures were randomized to receive either fully threaded screws (FT group, n=34) or partially threaded screws (PT group, n=34). The primary outcome was the American Orthopaedic Foot and Ankle Society (AOFAS) ankle hindfoot score at 12 months. Secondary outcomes included time to radiographic union and complication rates. Outcomes were assessed at regular intervals up to 12 months postoperatively. Overall, the results of the study revealed no statistically significant difference in AOFAS scores, time to union, or complication rates between groups. These findings suggest that both fixation methods yield comparable clinical and radiographic results for treating medial malleolar fractures.
