Results of a Standard versus an Accelerated Ponseti Protocol for Clubfoot: A Prospective Randomized Study.

One hundred infants (158 feet) with congenital club foot were randomized to undergo an accelerated Ponseti casting protocol performed twice-per-week or a standard Ponseti casting protocol performed once-per-week. Outcomes of interest included the Pirani and Dimeglio scores, the number of casts required, duration of casting, talocalcaneal and metatarsal angles, modified Functional Rating System (FRS) scores, and the incidence of adverse events. Outcomes were assessed upon completion of casting, then tri-montly until 1 year post-casting. Results revealed a significantly lower duration of casting in the accelerated group compared to the standard group. No significant differences in functional or biomechanical outcomes were observed between the two groups.
