Accuracy of Cup Positioning of Supine vs. Lateral Position in Total Hip Arthroplasty using Anterolateral Approach: A Prospective, Randomized, Controlled Trial

60 patients scheduled for total hip arthroplasty using an anterolateral approach were randomized to undergo the surgery in either the supine position or the lateral position. The purpose of this study was to compare mean cup inclination and anteversion between groups, as well as compare mean error from targeted inclination and anteversion angles. Target inclination was 40 degrees, while target anteversion was based relative to femoral stem anteversion observed intraoperatively. Mean error from targeted inclination was significantly lower in the supine group compared to the lateral group, whereas mean error from targeted anteversion did not significantly differ between groups. Rate of outliers (>10 degree error from targeted angles) was 3/29 in the supine group and 6/31 for the lateral group.
