2026 AAOS Annual Meeting: Kinematic and mechanical alignment result in similar outcomes in total knee arthroplasty: a systematic review and meta-analysis of randomized controlled trials

OE Journal. 2026;14(5):10Eleven randomized controlled trials including 972 patients undergoing total knee arthroplasty (TKA) were included in this systematic review and meta-analysis comparing kinematic alignment (KA) versus mechanical alignment (MA). Pooled outcomes included re-operation rates, component revision, objective clinical outcomes, and patient-reported outcome measures (PROMs). Overall, there was no statistically significant difference between KA and MA in re-operation rates or PROMs, although KA showed a non-significant trend toward higher re-operation risk.
