2025 AAOS Annual Meeting: Laminoplasty Versus Laminectomy and Posterior Fusion for Cervical Myelopathy: A Metaanalysis of Radiographic and Clinical Outcomes

Twenty-two studies involving 2,128 patients with cervical myelopathy were included in this systematic review and meta-analysis comparing laminoplasty (LP) versus laminectomy with fusion (LF). Pooled outcomes of interest included surgical factors (OR time, EBL, LOS), complications (overall, C5 palsy, reoperation), radiographic parameters (CL, cSVA, T1SA), and PROMs (NDI, VAS-neck, JOA). LP was associated with shorter operative time, lower blood loss, fewer complications, and better NDI scores. No significant differences were seen in other alignment or clinical outcomes. These findings suggest that LP offers a favorable perioperative and complication profile without compromising clinical or radiographic outcomes, supporting its use in appropriate patients.
