Clinical Effectiveness of Splinting After Collagenase Clostridium Histolyticum Injection for Dupuytren Contracture
Seventy-nine patients with Dupuytren contracture involving one or more digits were randomized to receive collagenase clostridium histolyticum followed by either night extension orthosis (n=39) or no orthosis (n=40). The primary outcome of interest was residual flexion contracture at 4 months. Secondary outcomes included residual contracture at earlier timepoints, QuickDASH scores, satisfaction, orthosis compliance, and adverse events. Outcomes were assessed at baseline, post-manipulation, 1 month, and 4 months. Overall, the results of the study revealed significantly lower residual contracture at 4 months in the orthosis group (11° vs 20°), particularly for PIP joints (19° vs 36°). Satisfaction was also higher in the orthosis group, though compliance declined over time. These findings suggest a meaningful benefit of night orthosis after CCH treatment, especially for PIP joint contractures.
