2024 AAHKS Annual Meeting: Use of Dexamethasone in Diabetic Patients Undergoing Total Joint Arthroplasty

Twelve studies involving diabetic patients undergoing total joint arthroplasty were included in this systematic review and meta-analysis comparing perioperative dexamethasone administration versus withholding dexamethasone. Pooled outcomes of interest included postoperative glucose levels, optimal dosing regimens, and complications such as infection. The administration of dexamethasone significantly elevated glucose levels on postoperative day 1 (POD1) but remained below the 200 mg/dL threshold, with no significant differences observed on POD2 or POD3. There was no increased risk of infection. The study concluded that dexamethasone is safe in diabetic patients, but further research is needed to optimize dosing and administration timing.
