2026 COA Annual Meeting: ''Doctor, How Long Will My Modern Hip Replacement Last?” Survivorship of Modern Total Hip Replacement to 30 Years

29 studies comprising of 5203 patients along with an analysis of 1.89 million total hip arthroplasties (THA) from 8 national joint registries were reviewed and evaluated for the survivorship of contemporary total hip replacements (THR). Modern bearing surfaces included highly cross-linked polyethylene combined with metal or third and fourth-generation ceramic heads, as well as ceramic-on-ceramic bearings. Literature-based analyses demonstrated survivorship rates of 96%–97% at 15 years, while registry data showed an overall survivorship of 93% at 20 years. Extrapolated survivorship estimates were 92% at 25 years and 91% at 30 years. These findings suggest that modern THR have substantially greater longevity than previously reported and may continue to function successfully for decades after implantation.
