High-dose dual-antibiotic loaded cement for hip hemiarthroplasty in the UK (WHiTE 8): a randomised controlled trial

4,936 patients aged 60 years or older, scheduled for a hemiarthroplasty surgery to treat hip fracture, were randomized to receive single-antibiotic loaded cement (n=2,453) or high-dose, dual-antibiotic loaded cement (n=2,483). The primary outcome of interest was the rate of deep surgical site infection at 90 days post-operation. Secondary outcomes of interest included mortality, antibiotic use, complications, quality of life, mobility, and the proportion of patients discharged home, assessed at 120 days post-operation. No differences in the rate of deep surgical site infection were observed between the single-antibiotic and high-dose, dual anti-biotic groups. Quality of life outcomes, mortality, complication rates, mobility, and the proportion of patients discharged home were also similar between groups. The results of the WHiTE 8 trial found no clinical benefit of the use of high-dose, dual-antibiotic loaded cement over single-antibiotic loaded cement for older patients undergoing hemiarthroplasty.
